Hospice Social Worker

Job Summary
Provide psycho-social support and counseling to patients and families within hospice care; collaborate with IDT to develop and implement the plan of care; conduct initial and ongoing psycho-social assessments; address financial, insurance, living arrangements, caregiver stressors, and community resource needs; facilitate placement in long-term care when warranted; support advance planning including living will/POA documents and funeral arrangements; maintain comprehensive care plans and coordinate with the interdisciplinary team to ensure continuity and quality of care.
Required Qualifications
- Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or other field related to social work with one (1) year of social work experience in a healthcare setting required; and licensure if required by state rules.
- Preferred - Master's degree in Social Work with one (1) year of social work experience in a healthcare setting highly preferred; and licensure if required by state rules.
Additional Requirements
- Missouri BSW from an accredited school of social work is minimum education required but must be supervised by MSW.
