Job Summary
Hospice Social Worker provides psychosocial assessments, counseling, and caregiver support as part of the interdisciplinary team (IDT). Responsibilities include explaining hospice services and Medicare benefits, timely documentation and care-plan development, conducting initial and ongoing psychosocial assessments, coordinating with external counseling resources, facilitating placement in long-term care when needed, participating in care planning meetings, supporting advance planning (living will/POA), and ensuring culturally respectful patient-family interactions. Requires MSW and state licensure as applicable; strong communication skills and ability to collaborate across healthcare settings. The role emphasizes nondiscrimination and a patient- and family-centered approach within a hospice and home-health care context.
Required Qualifications
- Master of Social Work (MSW) from a CSWE accredited program
- State licensure as required and applicable for the state in which you practice (LBSW, LMSW, LCSW, or LICSW)
- At least 1 year of healthcare experience (MSW track for supervision or bachelor’s degree in social work with 1 year healthcare experience considered for locations with MSW supervision)
