Job Summary
Deliver social work services to patients and families by assessing psychosocial function, developing care plans, and providing counseling related to health care services. Coordinate changes in patient care levels and location while actively participating in interdisciplinary team meetings. Supervise social work interns and maintain continuity of care through precise documentation in electronic medical records and structured hand-offs. Provide bereavement support, educate staff and community members on emotional dimensions of hospice, and utilize community resources to meet patient goals. Participate in weekday and weekend on-call rotations as assigned.
Required Qualifications
- Master's degree in Social Work from an accredited school
- Current LMSW license in the state of Maryland
- Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA and other federal, state and local laws
- Flexibility in schedule to manage frequency and duration of visits based on patient/family needs
- Must be able to lift and carry up to fifty (50) pounds with or without the use of assistive devices
- Understand and comply with Coastal Hospice's mission, values, vision, policies and Employee Handbook
Desired Qualifications
- 2 years' experience in hospital, long term care or hospice setting preferred
- LCWS-C preferred
