Hospice Social Worker

Assess patients' psychosocial needs and complete social histories, while documenting care plans, progress, and follow-up per regulations. Provide counseling and emotional support to residents and families, and support patients with adjustment, rights, and orientation. Coordinate referrals and act as a liaison with community resources, partnering with the care team to enhance quality of life. Plan and support patient, family, and community engagement activities, and promote facility programs to referral partners. Requires a Bachelor's Degree in Social Work, two years of verifiable experience, and hospice or palliative care experience preferred. Current driver's license, automobile liability insurance, and ability to travel between multiple locations within your region are necessary.
