Job Summary
Conduct psychosocial assessments within five days of admission, identifying emotional, social, financial, and environmental resources to inform the patient's plan of care. Participate in interdisciplinary and quality improvement meetings, observe and record changes in patient response to treatment, and maintain clinical records including daily notes. Evaluate intervention effectiveness, confirm weekly Case Manager visit scheduling, and assist the health team in understanding social and environmental factors. Act as a consultant to help patients utilize family and community resources while supporting families coping with the dying process. Requires a Licensed Independent Practitioner or Licensed Certified Social Worker licensed by Ohio, an MSW or accredited BSW with experience, and a valid driver's license. Hospice experience is preferred.
Required Qualifications
- Social Worker means an individual who is currently licensed to practice social work under the licensure categories of Licensed Independent Practitioner of Clinical Social Work, or Licensed Certified Social Worker or Licensed Social Worker. Must be Licensed by State of OHIO
- Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
- Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education
- one (1) year of social work experience in a healthcare setting
- Must be a licensed driver with an insured automobile in good working order
- The ability to communicate well, both verbally and in writing
- The ability to access patients' homes which may not be routinely wheelchair accessible is required
- Physical activities will include, walking, sitting, stooping, and standing
Desired Qualifications
- Assume responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading
- If BSW, one (1) year of Social Work experience mandatory; If MSW, one (1) year of Social Work experience preferred
- Hospice experience preferred
- Experience: Social Work: 1 year (Preferred)
