Job Summary
Hospice Social Worker providing psychosocial support and resources to home-based patients and their families. Responsibilities include psychosocial assessments, developing and maintaining a plan of care, supportive counseling, facilitating transitions, mobilizing community resources, patient/family education, documentation, patient safety, and collaboration with an interdisciplinary team. Role involves home visits with 50% caseload in home-based settings and requires driving and weekend availability. Requires MSW and 1-3 years of palliative/hospice experience; valid CO driver’s license and automobile insurance; strong communication and organizational skills; proficient computer skills; ability to work independently and manage time; bilingual skills considered a plus. Salary range is $33-$36/hour, Colorado statute, with pay calculated to annualized figures.
Required Qualifications
- Master's degree in social work from an accredited school
- minimum of 1-3 years of palliative care or hospice experience
- Current CO Driver's license and automobile insurance
- Excellent communication and interpersonal skills
- Ability to function independently and effectively anticipate needs and problems
- Proficient computer skills
- Ability to manage time well and perform duties with attention to detail and follow-through
- Ability to collaborate with interdisciplinary team in providing patient care
- Knowledge of self-care, personal boundaries; abilities, limits and inner resources
