Hospice Social Worker ($3,500 Sign-On Bonus)

Job Summary
Assess psychosocial status of patients and families regarding terminal illness, environment, and caregiver dynamics, communicating findings to the interdisciplinary care team. Conduct social evaluations covering family dynamics, suicide risk, and neglect, then counsel patients on stress and coping difficulties while providing crisis intervention. Coordinate with clinical staff to connect families with community resources, maintain clinical records, and assist in preparing advanced directives. Participate in interdisciplinary meetings to address psychosocial impacts on symptoms and assist in planning funeral arrangements and legal decisions. Provide ongoing bereavement support for high-risk individuals and manage support group schedules, ensuring compliance with state, federal, and CHAP regulations. Requires a current license (LBSW, LMSW, or LICSW), valid driver's license, and ability to lift up to 20 pounds. Master's degree preferred.
Required Qualifications
- Bachelor's degree in social work from an institution accredited by the Council on Social Work Education
- Current licensed in the state(s) of practice
- Three (3) licensure levels: Licensed Bachelor Social Worker (LBSW), Licensed Master Social Worker (LMSW), Licensed Independent Clinical Social Worker (LICSW)
- Social Workers can have compact licenses
- Ability to pass background, consumer report, and drug test
- Valid State Driver's License
- Access to an automobile in good working order
- Automobile insurance in accordance with state and organizational requirements
- Acceptable driving record
Desired Qualifications
- Master's degree in social work, preferred
- 1+ years of social work experience in a health care setting
- 1+ years of experience in a healthcare setting
