Hospice Social Services Team Lead

Job Summary
Lead and mentor a team of Licensed Social Workers by supervising performance reviews, coaching onboarding, and fostering a collaborative, patient-focused environment. Oversee psychosocial assessments, care planning, and crisis interventions while collaborating with nurses, physicians, and interdisciplinary members to support patients and families facing end-of-life challenges. Ensure compliance with Medicare Conditions of Participation, Arizona regulations, and HIPAA standards through documentation monitoring and quality improvement initiatives. Requires an MSW, active Arizona social work license, and three years of hospice experience.
Required Qualifications
- Master's Degree in Social Work (MSW) from a CSWE-accredited program
- Current, active Arizona social work license in good standing: LMSW or LCSW accepted
- Minimum of three (3) years of professional social work experience in hospice, palliative care, healthcare, home health, or a related clinical setting
- Minimum of one (1) year of leadership, supervisory, or management experience
- Strong understanding of psychosocial assessment, family dynamics, and end-of-life care
- Valid Arizona driver's license, reliable transportation, and current automobile insurance
Desired Qualifications
- Hospice experience strongly preferred
- Previous experience supervising social workers
- Knowledge of Medicare hospice regulations and interdisciplinary team practices
