Hospice RN - Weekend On-Call

Job Summary
Complete ongoing assessments of the patient's physical and psycho-social status, and initiate a plan of care in collaboration with the Interdisciplinary Group, patient, and attending physician. Provide education and interventions to manage pain and symptoms, then make necessary revisions to the plan as the patient's status changes. Ensure equipment, supplies, drugs, and biologicals are available, while completing accurate and timely documentation of all services provided. Regularly attend IDG meetings to discuss patient progress. This weekend-only role includes Fridays at 4:30 PM to Monday at 8:00 AM on-call. Candidates must hold an active, unrestricted Michigan RN license with at least one year of relevant experience.
Required Qualifications
- Registered Nurse
- Active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
- Weekends only
- Starting Fridays at 4:30PM - Monday at 8:00AM on-call
Desired Qualifications
- One (1) year of management experience
- One (1) year of home health experience
- Community health experience
- Wide range of Medical-Surgical experience
