Job Summary
Conduct ongoing assessments of patients' physical and psycho-social status, then initiate and develop care plans in collaboration with the interdisciplinary group, patient, and attending physician. Provide education and interventions for pain and symptom management while ensuring all equipment, supplies, drugs, and biologicals remain available. Make necessary revisions to the care plan as patient or family status changes and complete accurate, timely documentation of all services provided. Regularly attend interdisciplinary group meetings to coordinate care. This weekend-only role includes on-call availability from Fridays at 4:30 PM to Mondays at 8:00 AM. Candidates must hold an active, unrestricted Michigan RN license with at least one year of relevant experience, CPR certification, and reliable transportation.
Required Qualifications
- Registered Nurse
- Active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
- Weekends only
- Starting Fridays at 4:30PM - Monday at 8:00AM on-call
Desired Qualifications
- One (1) year of management experience
- One (1) year of home health experience
- Community health experience
- Wide range of Medical-Surgical experience
