Job Summary
Respond to patient and family calls during on-call hours, providing skilled nursing care, symptom management, and crisis intervention. Assess patient conditions, communicate with physicians and interdisciplinary team members, and coordinate emergency visits as needed. Document all interactions in the EMR system while supporting families with emotional and educational needs. Ensure continuity of care by following up with the primary hospice team and adhering to the assigned on-call rotation schedule. Travel locally to patient homes and hospice settings, managing stressful situations under pressure.
Required Qualifications
- Graduate of an accredited school of nursing
- Current RN license in the state of practice
- Minimum of 2 years of nursing experience
- At least 1 year in a hospice or palliative care setting
- Experience in crisis management and independent decision-making
