Job Summary
Coordinate physical care of patients with families, physicians, and the hospice team while applying nursing theories and concepts. Document care to meet reimbursement guidelines and communicate with patients, families, and all agency personnel. Attend and participate in IDT meetings and quality reviews as scheduled. Requires current RN licensure, two years of clinical experience, and one year of hospice experience preferred. Full-time weekend on-call rotation with acceptance of travel, evening, and holiday assignments. Background checks and drug-free workplace requirements apply.
Required Qualifications
- Current RN licensure in state of practice
- Successful completion of a nursing education from an approved school of nursing
- Two years of clinical nursing experience
- One year of hospice nursing experience
- Knowledge of the framework, function, and organization of a home health and hospice care program
- Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines
- 12 hours of continuing education annually
- CPR certification
- Accept travel based on agency need
- Accept weekend/evening and holiday call assignments
- Background checks/drug-free workplace
