Job Summary
Assess patients with advanced illnesses, create individualized hospice care plans, and provide education and emotional support to patients and families navigating end-of-life. Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care while documenting status updates and adjusting care plans as needs evolve. Complete a two-week orientation covering hospice practices, EMR systems, and safety protocols. This per diem role serves Monmouth County, offering a salary range of $43.00 to $51.00.
Required Qualifications
- RN with Bachelor's degree in Nursing
- 1+ year of supervised clinical experience as a registered nurse
- Valid NJ Driver's license
