Job Summary
Hospice RN visiting nurse (part-time, 0.6 FTE, 24 hours/week) providing scheduled visits and collaborating with the interdisciplinary team. Responsibilities include conducting physical, psychosocial and environmental assessments; updating the plan of care; following care plans; communicating changes to physicians and the primary RN; assisting with discharge planning; documenting according to guidelines; scheduling patient visits and case conferences; adhering to policies; ensuring patient safety and comfort, including patient positioning and safe lifting; requirements include RN licensure in the practicing state, BLS, Associate's Degree in Nursing, and typically 2 years of nursing experience. Must have strong communication, organizational, judgment, and multitasking abilities; physical stamina to meet life-span needs and occasional lifting/heavy activities; driving to Northwest suburbs as patient needs dictate.
Required Qualifications
- Registered Nurse license issued by the state in which the team member practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)
- Associate's Degree in Nursing
- 2 years of nursing experience
- Ability to lift up to 50 lbs., as described in job duties
