Job Summary
Hospice RN Visit Nurse providing after-hours home-based hospice care in the Twin Cities metro area. Responsibilities include completing initial client assessments, regular re-evaluations of nursing needs, developing and revising care plans, administering prescribed medications, counseling clients and families, coordinating with physicians and community health professionals, documenting care and updates, acting as Case Manager when assigned, ensuring effective communication within the team, and organizing travel and mileage documentation. Requires completing orientation/training, participating in Lifespark safety initiatives, and supporting caregiver supervision and patient education. Schedule is Friday–Sunday, 5pm–8am with on-call and rotating holidays; mileage reimbursement provided. Qualifications include 1–2 years of recent acute care experience, 1 year home health care preferred, nursing degree, Minnesota nursing license, and MVR check.
Required Qualifications
- Graduate of an accredited school of nursing
- Current Minnesota Nursing License
- Must pass a MVR check
