Job Summary
RN Alt Shift in PM shift providing hospice/palliative care with patient and family-centered nursing, care planning, pain management, and education. Responsibilities include patient and family assessments, developing and updating the plan of care (POC) with the Interdisciplinary Team (IDT), delivering interventions, documenting in EHR, coordinating with IDT, delegating visits to CNAs/LPNs, precepting new staff, participating in meetings, and ensuring quality hospice care across homecare teams. Includes on-call rotation, weekend and holiday rotations, and a $10,000 sign-on bonus. A current Wisconsin RN license, valid driver’s license, and ability to work 32 hours/week (full-time equivalent) are required.
Required Qualifications
- Current, unrestricted Registered Nursing License in state of employment
- Must be at least 18 years of age
- Valid driver’s license or access to a reliable vehicle
- Basic Microsoft Office skills
- Intermediate electronic health records skills
- Ability to use proper body mechanics and safe patient handling
Desired Qualifications
- Current, unrestricted Registered Nursing License in state of employment
- One-year nursing experience, preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Ability to use proper body mechanics, safe patient handling, and standard precautions
- Intermediate electronic health records skills
- Basic Microsoft Office skills
