Hospice RN (Registered Nurse, Visiting Nurse, Case Manager) - HomeCare

Conduct comprehensive assessments to establish goals for symptom management and end-of-life care with effective case management strategies. Support patients, families, and loved ones through their end-of-life journey by developing therapeutic relationships and utilizing state-of-the-art technology for documentation. Serve as a member of an active interdisciplinary team addressing patient needs holistically while providing personalized care in the home. This weekend position covers Friday, Saturday, Sunday, and Monday shifts. Candidates must hold a Connecticut RN license, an Associate's degree (BSN preferred), and one year of nursing experience in a specialty or home care setting.
