Hospice RN - Registered Nurse (Monroe/Loudon County)

Job Summary
The RN provides and directs the provision of nursing care to home-based patients as prescribed by the physician and in compliance with applicable laws and agency policies. Coordinate the total plan of care with other health care professionals involved in the care and strive to maintain continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources. Conduct the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, and develop the plan of care within state guidelines, ensuring patients meet home health eligibility and medical necessity guidelines defined by the payer. Maintain and submit accurate documentation and clinical notes detailing patient condition, goals, interventions, and response to care. Provide high-quality clinical services within the scope of practice as defined by state laws from admit through discharge. Work location is on the road serving Knoxville area and surrounding communities.
Required Qualifications
- One (1) year of clinical RN experience
- Current RN license in the state where services are rendered
- Current CPR certification
- Driver's License
