Hospice RN (Registered Nurse) Case Manager

Assess patient symptoms and collaborate with the attending physician and Medical Director to ensure effective symptom management. Evaluate treatment and medical conditions to individualize and adjust the plan of care as needs change. Manage interdisciplinary care by overseeing admissions, transfers, and discharges while supervising nurse aides and LPNs. Counsel patients and families on disease progression, incorporating them into plan development and listening to their concerns. Complete all clinical documentation adhering to agency protocol and Medicare/Federal guidelines. Attend in-service training and mandatory agency meetings. This role supports Summa Hospice's mission to provide high-quality end-of-life care in patients' homes, aligning with industry best practices and emphasizing staff development.
