Hospice RN (Registered Nurse) Case Manager

Manage interdisciplinary care for hospice patients by conducting thorough symptom assessments and collaborating with attending physicians and the Medical Director to ensure effective symptom management. Evaluate and individualize treatment plans according to established protocols, adjusting care as patient needs change while overseeing the total care provided by nurse aides and LPNs. Counsel families on disease progression and actively incorporate their input into the development of personalized plans of care. Complete all clinical documentation adhering to agency protocol and Medicare/Federal guidelines while promoting continuity during admissions, transfers, and discharges. Work within an interdisciplinary team to deliver an excellent end-of-life experience, attending mandatory in-service training and agency meetings.
