Hospice RN, PRN

Administer nursing care and skills to homebound patients while establishing treatment plans according to the Plan of Care. Monitor patient symptoms or behavior, manage multi-disciplinary care for continuity of admissions, transfers, and discharges, and communicate with the Interdisciplinary team. Educate patients and families on health maintenance, provide emotional support, and document visits promptly in the electronic charting system. This PRN role covers Richmond and surrounding areas with flexible scheduling and per-visit compensation ranging from $32 to $45 per hour.
