Job Summary
Provide home-based hospice care to patients after-hours, weekdays, weekends, and as needs arise. Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications, and ensure accessibility of medical equipment and supplies. Educate families on prognosis, care plan changes, and hospice goals while maintaining accurate records. Work as part of an interdisciplinary team to carry out the patient's plan of care and triage emergent needs. Maintain 1:1 nurse-to-patient ratios and provide services in accordance with agency policies. Requires RN license, CPR certification, valid car insurance, and 1+ years of prior hospice experience.
Required Qualifications
- Registered Nurse (RN) license
- CPR certification
- Valid car insurance
- Prior Hospice experience 1+ years
- Strong critical thinking and problem-solving skills
- Excellent communication and interpersonal skills
- Proficiency in medical terminology and understanding of Hospice nursing care
- Ability to work independently and manage time effectively
