Job Summary
Assess patients' physical condition, develop individualized care plans, and monitor vital signs while administering medications and treatments as prescribed. Manage pain and symptoms using palliative care techniques, assist with activities of daily living, and collaborate with physicians, social workers, and interdisciplinary team members for care planning. Educate patients and families on disease progression, provide emotional and spiritual support, and guide families on caregiving techniques and available resources. Maintain accurate clinical documentation, ensure compliance with hospice regulations, and respond to urgent patient needs or after-hours calls. Notify appropriate parties at time of death and complete post-mortem procedures.
Required Qualifications
- Graduate of an accredited nursing program (ADN or BSN)
- Current Registered Nurse (RN) license in the state
- CPR certification
- 1–2+ years nursing experience
- Ability to work independently in home or facility settings
- Documentation and EHR proficiency
- Flexible scheduling, including evenings, weekends, or on‐call shifts
Desired Qualifications
- Hospice and Palliative Care certification
- Experience with end‐of‐life care and symptom management
- Experience in hospice, home health, or acute care
- Knowledge of hospice philosophy and palliative care practices
- Strong clinical assessment and critical thinking skills
- Compassion and emotional resilience
- Excellent communication and interpersonal skills
