Hospice RN PRN

PRN Hospice Registered Nurse to support Hermitage and surrounding counties. Responsibilities include assessement and management of patient and family caseload, determination of eligibility, admission processing, planning, implementation and evaluation of the nursing process; obtain data from patient and family across physical, psychological, social and spiritual domains; coordinate care with attending physician and interdisciplinary team; provide home visits to reassess needs and update the plan of care; perform nursing procedures as directed; perform clinical pronouncement of death and provide support to family; maintain accurate patient records, charts, progress notes, reports and care plans.
