Job Summary
On-Call RN Case Manager for Lifted Hospice delivering in-home end-of-life nursing care. The role leads bedside care, assesses patient needs, manages symptoms, and collaborates with the interdisciplinary team to develop and follow care plans that align with patient goals and family needs. Responsibilities include thorough assessments, symptom management, maintaining accurate documentation, and educating families to support comfort and understanding during end-of-life care. Requires graduation from an accredited nursing program (diploma/associate/bachelor’s), active state license, prior RN experience in clinical settings (hospice preferred), strong communication, English literacy, and valid driver’s license with reliable transportation.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program
- Active state nursing license
- Clinical skills that meet current standards of nursing practice
- Experience as a registered nurse in a clinical care setting
- Hospice experience preferred
- Excellent interpersonal skills
- Must read, write and comprehend English
- Current driver’s license, reliable transportation, and valid auto liability insurance
