Job Summary
Assess, plan, implement, and evaluate nursing care for hospice patients, focusing on pain control, symptom management, and end-of-life needs. Initiate communication with physicians, the Medical Director, and the interdisciplinary team to coordinate care plans and eligibility determinations. Administer medications per federal, state, and agency policies while maintaining accurate, real-time documentation of problems, interventions, and charges. Counsel patients and families on end-of-life needs and perform comprehensive death notes when appropriate. Maintain current patient records in accordance with Medicare regulations and agency policy. Requires a state-certified RN license, hospice or home health experience, and local travel availability.
Required Qualifications
- Associate's Degree in Nursing
- State Certified RN license
- Experience in either Hospice or home health care
- Must have excellent documentation skills
- Must have the ability to work independently
- Must have strong communication skills
- Valid driver's license
- Reliable transportation
- Must be available to travel locally within assigned territory
