Hospice RN [On-Call Nights & Weekends ]- West Valley

Evaluate patient eligibility and re-certification criteria, then assess the impact of serious illness on physical, functional, environmental, safety, and psycho-social needs. Develop individualized pain management plans and palliative care strategies to manage discomfort and provide symptom relief. Identify verbal and non-verbal communication patterns and evaluate caregiver ability to meet immediate patient needs. This on-call position covers Friday through Sunday for the Phoenix Hospice Team. Compensation includes $50 per PM visit on weekdays and $100 on weekends, with separate rates for routine and assisted visits. Candidates must hold an active Arizona nursing license, current CPR certification, and TB clearance. Assisted offers comprehensive health coverage, a 401(k) match, and tuition reimbursement.
