Job Summary
Provide home-based care to patients with six months or less to live, focusing on quality of life. Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments, and ensure accessibility of medical equipment and supplies. Work as part of an interdisciplinary team to carry out the patient's plan of care while educating families on prognosis, care changes, and hospice goals. Take a holistic approach to care that engages the patient's mind, body, spirit, and family. Requires an active, unrestricted Michigan RN license with at least one year of relevant experience, reliable transportation, and a valid driver's license. Schedule is Monday through Thursday from 4:30pm to 8:00am.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN

- Community health and a wide range of Medical-Surgical experience
