Job Summary
Provide home-based hospice care to patients with six months or less to live, performing assessments, vitals checks, pain monitoring, and administering prescribed medications. Work within an interdisciplinary team to execute patient care plans while educating families on prognosis, care changes, and hospice goals. Ensure accessibility of medical equipment, oxygen, and supplies while taking a holistic approach that engages the patient's mind, body, spirit, and family. Maintain 1:1 nurse-to-patient ratios with flexible assignments and autonomy. Requires active Michigan RN license, one year of experience, reliable transportation, and valid driver's license. Monday through Thursday schedule from 4:30pm to 8:00am. Competitive weekly pay of $72k-$75k.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN

- Community health and a wide range of Medical-Surgical experience
