Job Summary
Conduct home-based hospice care for patients with six months or less to live, performing assessments, vital checks, and administering prescribed medications. Collaborate with an interdisciplinary team including physicians, social workers, and chaplains to execute individualized plans of care while educating families on prognosis and goals. Maintain accessibility of medical equipment, oxygen, and supplies while taking a holistic approach that engages the patient's mind, body, and spirit. Requires an active Michigan RN license with one year of experience, reliable transportation, and a valid driver's license. Schedule runs Monday through Thursday from 4:30pm to 8:00am with weekly pay ranging from $72k to $75k.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN
