Job Summary
Provide home-based hospice care to patients with six months or less to live, performing assessments, vital checks, pain monitoring, and administering prescribed medications. Collaborate with an interdisciplinary team including physicians, social workers, and volunteers to execute patient care plans while educating families on prognosis and goals. Ensure accessibility of medical equipment, oxygen, and supplies while maintaining a holistic approach that engages the patient's mind, body, spirit, and family. Requires an active Michigan RN license with at least one year of experience, reliable transportation, and a valid driver's license. Schedule runs Monday through Thursday from 4:30pm to 8:00am with weekly pay ranging from $72k to $75k.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN

- Community health and a wide range of Medical-Surgical experience
