Job Summary
Provide home-based hospice care to patients with six months or less to live, performing assessments, vital checks, and administering prescribed medications. Work within an interdisciplinary team to execute patient care plans, educate families on prognosis and goals, and ensure accessibility of medical equipment and supplies. Maintain a holistic approach that engages the patient's mind, body, spirit, and family. Requires an active Michigan RN license with one year of experience, reliable transportation, and a valid driver's license. Schedule runs Monday through Thursday from 4:30pm to 8:00am with weekly pay ranging from $72k to $75k.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN

- Community health and a wide range of Medical-Surgical experience
