Job Summary
Provide home-based care to patients with six months or less to live, performing assessments, vital checks, and administering prescribed medications. Work within an interdisciplinary team to execute patient care plans, ensuring accessibility of medical equipment and supplies. Educate families on prognosis, care changes, and hospice goals while taking a holistic approach to engage the patient's mind, body, spirit, and family. Requires an active, unrestricted Michigan RN license with one year of relevant experience, reliable transportation, and a valid driver's license. Schedule is Monday through Thursday from 4:30pm to 8:00am with weekly pay ranging from $72k to $75k.
Required Qualifications
- RN with active, unrestricted license in the State of Michigan
- At least one (1) year of relevant experience as an RN

- Community health and a wide range of Medical-Surgical experience
