Job Summary
On-Call Registered Nurse (RN) Case Manager delivering hospice care in patients' homes, coordinating with an interdisciplinary team to build and follow care plans honoring patient goals, educating families, and documenting care to ensure high standards of clinical and emotional support. Responsibilities include thorough assessments, symptom management, collaboration with the care team, and ensuring families are prepared and confident in caregiving within the sacred context of end-of-life care.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program
- active state nursing license
- clinical skills meeting current standards
- experience as a registered nurse in a clinical care setting
- Hospice experience preferred
- excellent interpersonal skills
- must read, write and comprehend English
- current driver’s license, reliable transportation, and valid auto liability insurance
