Hospice RN Nurse Case Manager - $110k

Nurse Case Manager role providing in-home and facility-based visits across the Greenville/Anderson/Pickens/Abbeville SC service area. Responsibilities include facilitating receipt and review of medical records, enrolling patients in Care Management, initiating and updating Care Plans of Care, capturing diagnoses with high specificity, completing AWVs and cognitive/SDoH assessments, coordinating telehealth visits, and collaborating with an interdisciplinary care team to ensure safe, quality patient care. Requires reliable transportation and daily travel to patients' homes and facilities; 12-hour shifts; strong communication skills and ability to coordinate with providers, therapists, and caregivers. RN licensure in good standing is required; a minimum of three years clinical experience is preferred.
