Hospice RN Clinical Manager

Job Summary
Coordinate patient care by receiving referrals, reviewing case information including home visits, and assigning hospice personnel. Evaluate cases through record reviews and home visits, instruct clinicians to promote quality home care services, and conduct quarterly record reviews. Assist in establishing therapeutic goals, developing plans of care, and attending interdisciplinary case conference meetings. Manage medical records in the EMR system and support the Clinical Director in planning continuing education programs and organizational policies. Participate in quality assessment performance improvement teams and community activities.
Required Qualifications
- RN
- hospice experience
- current licensure to practice professional nursing in the state
- BSN degree
- one (1) year experience as a professional nurse within the last three (3) years
- Previous experience in hospice/home care setting
- two (2) years management or supervisory experience
- Proven ability to work within an interdisciplinary setting
- excellent observation
- good nursing judgment
- communication skills
- Understands hospice philosophy and issues of death/dying
- Understands principles of pain/symptom management
- Must be a licensed driver
- automobile that is insured in accordance with state and/or organization requirements
- automobile in good working order
- current CPR certification
Desired Qualifications
- Graduate of NLN accredited school of nursing
- HCHB and Pointcare experience
