Job Summary
Plan, organize, and direct hospice care services from initial evaluation through pain and medication management, developing comprehensive care plans that are revised based on patient needs. Prepare complete clinical notes, coordinate delivery with an interdisciplinary team, and collaborate with primary care physicians to ensure continuity. Provide coaching and education to patients and caregivers to empower self-management and improve quality of life. Cover San Mateo and San Francisco on weekends as a full-time Registered Nurse Case Manager.
Required Qualifications
- Current CA Registered Nurse (RN) license
- Current CPR card for Healthcare Providers
- 1 year of Registered Nursing experience
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- Excellent observation skills
- Problem-solving skills
- Verbal and written communication skills
- Nursing skills per competency checklist
- Self-directed work style
- Ability to work with minimal supervision
- Full-Time Employment status
- Able to cover San Mateo and San Francisco on weekends
