Job Summary
Hospice RN Case Manager position in the Wayne County/Detroit area. Provides skilled nursing care and compassionate support to patients and families at end of life, coordinating comprehensive care with an interdisciplinary team across patients' homes, assisted living, and nursing facilities. Build rapport with patients, families, physicians, and facilities; manage a caseload with autonomy to address physical, emotional, and spiritual needs. Daily travel within the service area is required to deliver care in patients' homes. Typical hours are Monday–Friday, 8 a.m.–5 p.m., with a weekend on-call rotation. Eligible for PTO, comprehensive benefits, mileage reimbursement, a company vehicle program for field roles, and other perks. Background check required; equal opportunity employer.
