Job Summary
Provide skilled nursing care to hospice patients in home settings, including administration of medications and treatments per the physician's plan of care; perform comprehensive assessments and coordinate the plan of care with the interdisciplinary team and family; monitor and report patient status to the nursing supervisor and physician; participate in weekly interdisciplinary team meetings; maintain documentation in the agency's electronic system in a timely manner; support hospice aide evaluation as needed; weekend and holiday coverage as needed. Required Qualifications
- Graduate of an accredited RN program
- Registered Nurse Licensed in the state of employment
- At least 1-2 years of RN experience is a MUST
- Previous Hospice care strongly desired
- Basic knowledge of iOs operating system
