Job Summary
Manage a hospice nursing caseload across Victoria-area communities by completing assessments in patients' homes and building individualized care plans with interdisciplinary partners. Teach families about pain control, symptom management, and medications while coordinating comfort-focused support to ensure timely care. Maintain accurate EMR documentation and participate in a shared weekday and weekend on-call rotation. This full-time field-based role serves Victoria, TX, with a Monday–Friday schedule from 8:30 AM to 5:30 PM, requiring candidates to live within reasonable distance.
Required Qualifications
- Active Texas RN license or valid multistate RN compact privilege to practice in Texas
- Able to work field-based across Victoria-area communities
- Comfortable supporting patients and teaching families during difficult, emotional moments
- Able to manage documentation accurately in an EMR
- Ready to participate in both weekday and weekend on-call rotation
- Organized enough to manage a caseload while moving between homes and care settings
- Must live within reasonable distance of Victoria, TX to support field visits and on-call needs
