Hospice RN Case Manager

Hospice RN Case Manager provides professional nursing care for assigned patients across adult, pediatric, and perinatal populations. Collaborates with the interdisciplinary team (IDT), patient, and family to assess, develop, implement, and evaluate the plan of care; ensures effective communication and care coordination with internal/external stakeholders; travels to patient visits in North San Diego County and South Riverside County; on-call coverage required monthly with weekend shifts. Travel is required. Operates within a nonprofit hospice framework serving San Diego County and south Riverside County; dedicated to improving quality of life at end of life and supporting families.
