Job Summary
As a Hospice RN Case Manager in Optimal Care’s talent pool, you will provide skilled nursing care and compassionate support to patients and families in their homes, assisted living facilities, or nursing homes within the Carmel, IN service area. You will build report with patients, families, physicians, facilities, and the interdisciplinary team; coordinate comprehensive care across settings; assess and respond to patients’ physical, emotional, and spiritual needs; manage a caseload with strong documentation and time-management skills; and travel daily within the service area to deliver high-quality hospice care. This role requires RN licensure, 1+ year clinical experience, valid driver’s license and auto insurance, and the ability to work within a team to deliver patient-centered care under a flexible schedule (Full Time, Part Time, or Per Diem).
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum one (1) year experience as a nurse in a clinical care setting
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
