Hospice RN Case Manager (St. Croix & Dunn County Area) 1.0 Exempt

Job Summary
Conduct comprehensive assessments within five days of admission and develop individualized plans of care with interdisciplinary team members. Provide skilled nursing care, teaching, and medication management to patients and families in their residences while ensuring physician orders are implemented timely. Maintain accurate documentation within 24 hours of visits, update status reports to physicians every 14 days, and notify the team of patient deaths by 9 AM the following business day. Supervise home health aides biweekly, coordinate care with other disciplines, and complete discharge summaries within one week of death. Work full-time in St. Croix and Dunn County areas, rotating on-call responsibilities, with a minimum of two years of hospice experience required.
Required Qualifications
- BSN or minimum of two years of hospice care experience
- Minimum of two years of clinical experience
- Current Wisconsin RN license
- Current driver's license
- Able to take on call responsibilities on rotating basis
- High level of attention to detail
- Ability to work autonomously and make independent judgments
- Ability to manage conflicting priorities and handle multiple tasks/projects concurrently
- Ability to work well as part of a team and independently
- Exhibit excellent written and verbal communication and organizational skills
- Exceptional customer service skills
- Display and promote high standards of ethical conduct and behaviors consistent with organizational standards
- Job involves standing more than four hours per day
- Stooping, bending, lifting/pushing up to 25 lbs
