Hospice RN Case Manager - South Tampa

Manage patient discomfort and provide symptom relief under physician direction using specialized nursing skills for palliative and end-of-life care. Conduct initial and ongoing assessments of terminal diagnosis impacts on physical, functional, psychosocial, and environmental needs. Collaborate with the Interdisciplinary Group to develop, implement, and update individualized care plans, initiate preventive procedures, and coordinate referrals. Educate patients, families, and caregivers regarding disease processes, palliative interventions, and safety practices. Requires current unrestricted Florida RN license, one year acute-care hospital experience, and Monday-Friday 8a-5p schedule.
