Hospice RN Case Manager - Sign on Bonus Eligible

Hospice RN Case Manager providing in-home hospice and residents’ families with a multidisciplinary team approach. Responsibilities include assessing patient preferences and barriers to involvement in care, developing and documenting individual care plans with defined goals in collaboration with an interprofessional team and patient/family, proactively communicating the plan of care across the continuum, and ensuring care quality and continuity. The role emphasizes patient- and family-centered care, respect, equity, and empathy across diverse populations, with duties including updating plans in the EMR, coordinating care, and advocating for patients and families. Requires nursing licensure (RN) and an appropriate nursing education background; on-call and weekend coverage are included, with relocation available and a sign-on bonus potential.
