Hospice RN Case Manager / RNCM (South OC)

Complete initial patient and family assessments to determine home care needs, including physical exams and illness histories. Initiate and revise plans of care based on changing patient status, utilizing health data to establish nursing diagnoses and develop goals. Implement preventive and rehabilitative nursing procedures, administer prescribed medications, and counsel patients on meeting care objectives. Provide health care instructions and identify discharge planning needs to ensure safe transitions. Requires current RN license, acute care experience, and BLS certification. Full-time day shift in South OC territory.
