Hospice RN Case Manager / RNCM

Conduct initial patient and family assessments to determine home care needs and administer prescribed medications and treatments. Initiate and revise plans of care based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative actions while involving patients and families in planning. Re-evaluate patient needs regularly, provide health care instructions, and identify discharge planning requirements to implement prior to patient discharge. Requires current RN license, BLS certification, and valid driver's license. Schedule is Monday-Friday, 8:00am-5:00pm, with hourly pay ranging from $50.00 to $60.00.
