Hospice RN Case Manager

Hospice RN Case Manager provides intermittent visiting nursing care, assesses and develops comprehensive patient care plans, and coordinates with an interdisciplinary team. Responsibilities include planning, implementing, and evaluating total nursing care, supervising LPN/LVN and Home Health Aides, ensuring timely documentation and certifications, and educating patients and families in end-of-life care and disease processes. Utilizes critical thinking and communication to address physiological, psychosocial, and cultural needs and collaborates with physical, speech, or occupational therapy and medical social services within Riverside Health System.
