Job Summary
Conduct comprehensive nursing assessments, care planning, and clinical oversight for hospice patients and families. Coordinate with physicians for terminal illness certification and recertification per Medicare regulations, serving as the clinical point person for symptom escalation and advanced care planning. Develop individualized care plans with patients, families, and the interdisciplinary team while supervising hospice aides. Participate in interdisciplinary meetings, manage on-call rotations for urgent needs, and ensure documentation meets regulatory and billing compliance standards.
Required Qualifications
- Current RN license in the state of practice or valid multistate compact license
- CPR certification
- Valid driver's license
- Graduate of an accredited nursing program
- Ability to work independently while collaborating across interdisciplinary teams
- Excellent communication, critical thinking, and organizational skills
Desired Qualifications
- Associate degree or higher
- Minimum one (1) year of hospice, palliative care, or home health experience
- Knowledge of hospice regulations and Medicare Conditions of Participation
