Hospice RN Case Manager Part Time El Centro

Oversee hospice patient care management by conducting comprehensive initial and ongoing assessments to determine patient needs. Develop and revise individualized care plans that incorporate palliative nursing actions while administering prescribed medications and treatments. Coordinate patient care for assigned caseloads and communicate regularly with physicians and interdisciplinary teams to report condition changes and obtain orders. Provide symptom management, end-of-life education, and health counseling to patients and caregivers, including teaching self-care techniques. Maintain accurate clinical notes regarding pain management and care outcomes. Participate in on-call duties, attend interdisciplinary group meetings, and engage in quality assessment performance improvement activities. Requires active RN license, one year of acute care experience, and valid driver's license.
